La Obra Social posee distintas categorías afiliatorias las cuales se detallan a continuación.
Para acceder a los servicios y prestaciones de salud, los beneficiarios deben realizar previamente el correspondiente trámite de alta.

- AFILIADOS OBLIGATORIOS: comprende los funcionarios y agentes del Estado provincial y su grupo familiar primario, como así también los beneficiarios de la Caja de Jubilaciones y Pensiones de la Provincia de Santa Fe. Los docentes de escuelas provinciales tienen distintos grados de cobertura en el IAPOS, según su situación de revista.

- AFILIADOS POR ORGANISMOS ADHERIDOS: incluye los empleados de Municipios y Comunas de la provincia que hayan suscripto convenio con la obra social y sus respectivas Cajas de Jubilaciones y Pensiones, además de los trabajadores de instituciones con personería jurídica, gremial, mutual y análogas que tengan convenio con el Instituto. Incluye la afiliación de ex-presos políticos y su grupo familiar primario, y las madres de hijos desaparecidos.

- AFILIADOS VOLUNTARIOS: se pueden adherir a familiares del titular que se encuentren fuera del grupo familiar primario cuando cumplan determinados requisitos preestablecidos.

- AFILIADOS POR SISTEMA ABIERTO: abarca a quienes hayan sido titulares de la obra social y deseen continuar con la cobertura de IAPOS.

- OTRAS AFILIACIONES: pueden acceder a la afiliación por jubilación o pensión en trámite quienes iniciaron su trámite de jubilación o pensión en la Caja de Jubilaciones y Pensiones de la Provincia de Santa Fe o de Municipios o Comunas adheridos. Por otro lado, los afiliados que posean el Certificado Único de Discapacidad pueden tramitar la credencial como afiliado con discapacidad. En el caso de hijos con discapacidad, una vez realizado este trámite, le permite mantener la continuidad afiliatoria después de cumplir los 21 años.
